The Five Guys restaurant is an American Fast casual Restaurant chain. It has been serving handcrafted hamburgers and fries since 1986. The menu consists of Hamburgers, French fries and Hot Dogs. The company owns over 1,000 locations open throughout the United States and Canada. The menu consists of Hamburgers made with  American cheese or Bacon, Kosher style Hotdog, Grilled cheese and organic vegetables.

The buns are sweeter than normal buns. French fries are the sole side item, which is salted. Roasted in-shell peanuts are available here. There are more than 1,000 locations across the country and 1,500 outlets in development. The company added coffee in the menu, but this was discontinued due to quality concerns.

• Indicate your contact information for sweepstakes entry purposes.

• Specify whether you would like Five Guys to contact you in future for research or promotional purposes.

• Answer the remaining questions and submit the survey.
