• A valid store receipt with the Kmart Customer Survey CODE.

• A computer, Laptop, tablet or a mobile device with smooth internet access.

• Basic understanding of the English or Spanish language.

Kmart Survey Rules

•  You must be at the age of 18 years at the time of taking the survey.

• You must be a legal resident of the 50 United States and The District of Columbia.

• A purchase will not boost your chance of winning the Kmart Survey sweepstakes.

• You are allowed to take only one survey per each purchase receipt.

• More than two members of the same family are not able to win the prize twice.

Instructions

• Go to www.Kmartfeedback.com.

• Click the button for English or Spanish, whichever you prefer.

• Enter your 19-digit receipt number from your receipt. There is a diagram beside the boxes to show you where to look. Click next.

• You will be asked a series of questions about your recent visit. Please answer these questions as honestly and thoroughly as possible. You have to answer all questions on one page before moving on.

• At the end of the survey, you will be asked to enter information into a form in order to enter the gift card drawing. Please follow directions. If you do not wish to enter the sweepstakes, please close your browser window. Your feedback will still be sent.
